MEDIA RELEASE - If agreed at registration, I give permission for images of myself /my child being used for the promotion and advertising of water polo and /or Balmain Water Polo Club in mediums such as our website or our social media platforms or Water Polo NSW media platforms, local and metropolitan newspaper or television media. 

CODE OF CONDUCT - I have read (my and each of my team members has read) the UTS BWPC 12s Carnival Code of Conduct and agree to abide by it. 

TERMS OF REGISTRATION - By agreeing to Balmain Water Polo Club Terms of registration, I agree:

• To uphold the values and good standing of the club, and importantly - to respect all club and association officials; and that
• I understand that water polo is a strenuous physical activity.
• In the case of an emergency, you authorise us, where it is impracticable to communicate with a suitable adult, to arrange for the registered participant to receive such medical or surgical treatment as may be deemed necessary. You undertake to pay or reimburse costs which may be incurred for medical attention, ambulance transport and drugs while the registered participant is registered with the club.
• I understand that although Balmain Water Polo Club and its service providers attempt to minimise any risk of personal injury within practical boundaries, accidents do happen and you acknowledge that there is an inherent risk of personal injury in physical activities that will be undertaken as registered participant.
